Indications

Primary Indications

Cognitive impairment due to cerebrovascular insufficiencyStroke recoveryChronic cerebral ischemia

Mechanism

Inhibits phosphodiesterase type 1 (PDE1), leading to vasodilation in cerebral blood vessels; enhances oxygen and glucose utilization in the brain; has antioxidant and neuroprotective properties.

Dosing

Adult Dosing

oral: Typical dose: 5–10 mg three times daily

intravenous: 20 mg/day for 10–14 days (outside US, followed by oral maintenance)

Dosage Forms

  • Tablets: 5 mg, 10 mg
  • Solution for injection (outside US)

Onset Duration

Clinical effects on cognition may require several weeks of use; vasodilatory effects occur within hours of dosing.

Adverse Effects

• dry mouth
