Narrative Exposure Therapy (NET)

Reviewed by the HeyPsych Medical Review Board

Board-certified psychiatrists and mental health professionals

Indications

Primary Indications

PtsdComplex PtsdTrauma Related Disorders

Mechanism

NET is based on the principles of exposure therapy and narrative construction. Patients are guided to construct a detailed chronological account of their life, weaving traumatic memories into a coherent narrative. By doing so, fragmented traumatic memories become contextualized and integrated, reducing their intrusive and overwhelming quality. The process strengthens autobiographical memory, provides meaning-making, and promotes identity continuity.

Protocol

Preparation

Assessment of trauma history, stabilization of acute safety concerns, and psychoeducation about trauma and memory.

Procedure

  1. Therapist guides the patient through a chronological life review.
  2. Traumatic events are revisited in detail, with emotional processing and contextual integration.
  3. Positive and neutral events are interwoven to provide balance.
  4. A written narrative is often created and reviewed together.

Frequency: Weekly or bi-weekly sessions.

Duration: 8–16 sessions, depending on trauma complexity.

Total Treatment Time: 2–4 months typical.

Session Structure

Pre-Session

Check-in, review of emotional stability and safety.

Treatment Phase

Narrative construction and detailed exploration of traumatic events with therapist support.

Post-Session

Review of progress, grounding exercises, planning for self-care between sessions.

Expected Outcomes

Immediate

  • Increased emotional arousal during sessions
  • Emergence of coherent life story

Short Term

  • Reduced intrusive trauma symptoms
  • Improved mood stability

Long Term

  • Integrated autobiographical memory
  • Reduced PTSD symptoms
  • Improved identity continuity

Side Effects

common

  • Temporary distress
  • Heightened anxiety when recounting trauma

rare

  • Re-traumatization if not adequately supported

Contraindications

absolute

  • Active psychosis
  • Severe cognitive impairment preventing narrative construction

relative

  • Severe dissociation requiring prior stabilization
  • Unstable living environment without safety

Patient Selection

ideal candidates

  • Survivors of multiple or cumulative traumas
  • Refugees and displaced populations
  • Individuals with PTSD who can tolerate exposure-based therapy

screening required

  • Trauma history assessment
  • Evaluation for dissociation and safety risks

Training Requirements

practitioner

  • Mental health professionals trained in NET protocols

facility

  • Clinic, private practice, or community-based settings

Research Evidence

Key Studies

  • Schauer, M., Neuner, F., & Elbert, T. (2011). Narrative Exposure Therapy: A Short-Term Treatment for Traumatic Stress Disorders.
  • RCTs show effectiveness for PTSD in refugees, torture survivors, and war-affected populations.

Limitations

Less studied in single-incident trauma; requires emotional readiness for exposure.

Cost Considerations

typical session cost: $100–$200

total treatment cost: $800–$3,200 depending on duration and setting

insurance coverage: Often covered under psychotherapy benefits for PTSD

cost effectiveness: Efficient due to short-term duration and strong outcomes in complex trauma
