Loprazolam

Indications

Primary Indications

Short-term treatment of severe insomniaSituational sleep disturbances

Mechanism

Enhances the effect of GABA at the GABA-A receptor complex, increasing chloride influx into neurons and producing sedative and anxiolytic effects.

Dosing

Adult Dosing

insomnia: 1–2 mg orally at bedtime; maximum 2 mg/day

Dosage Forms

  • Tablets: 1 mg, 2 mg

Onset Duration

Onset within 30 minutes; duration of action about 6–8 hours.

Adverse Effects

Common Side Effects

• drowsiness
• dizziness
• headache
• anterograde amnesia

⚠️ Serious Side Effects

  • respiratory depression
  • severe allergic reaction
  • dependence and withdrawal

Warnings

Critical Safety Information

  • →Risk of dependence and withdrawal with prolonged use
  • →May cause complex sleep-related behaviors (e.g., sleep-driving)
  • →Avoid alcohol and other CNS depressants

Interactions

With: Alcohol

Risk: Enhanced CNS depression

Action: Avoid combination

With: Opioids

Risk: Severe sedation and respiratory depression

Action: Avoid or monitor closely

With: Other CNS depressants

Risk: Additive sedative effects

Monitoring

  • Level of sedation
  • Respiratory rate in at-risk patients
  • Signs of misuse, abuse, or dependence

Special Populations

👶Pregnancy

Avoid unless benefits outweigh risks; risk of neonatal withdrawal or floppy infant syndrome

🤱Breastfeeding

Excreted in breast milk; avoid use

👧Children & Adolescents (Under 18)

Not recommended

👴Older Adults (65+)

Increased sensitivity to sedatives; start at lowest dose

Tapering

Gradually reduce dose over weeks to prevent withdrawal symptoms such as anxiety, insomnia rebound, tremors, or seizures.

Clinical Notes

  • Intended for short-term use (≤4 weeks) to minimize dependence risk
  • Lower risk of next-day drowsiness compared to some longer-acting benzodiazepines
  • Efficacy may diminish with repeated use due to tolerance
