Lamotrigine crosses the placenta. During pregnancy, lamotrigine levels can drop significantly due to physiologic changes, which may require gradual dose adjustments and periodic blood level monitoring. After delivery, levels can rebound back toward baseline within weeks, so a plan to return to pre-pregnancy dosing helps reduce toxicity risk. In bipolar disorder, untreated illness also carries real risks, so decisions should be shared, individualized, and stability-focused rather than fear-based.🤱Breastfeeding
Lamotrigine is present in breast milk, and infant serum levels can be measurable. Some infants exposed via breast milk have had drowsiness, poor feeding, poor weight gain, apnea, or rash. Many infants do well, but monitoring matters—especially in newborns and young infants who clear the drug more slowly.👧Children & Adolescents (Under 18)
For psychiatric use, safety and efficacy for bipolar disorder in pediatric patients are not established in the same way as adult bipolar maintenance. Pediatric use is primarily in seizure disorders and requires careful titration due to rash risk.👴Older Adults (65+)
Older adults may be more sensitive to dizziness and fall risk. Use cautious titration, review other meds that increase falls, and consider additional cardiac risk screening when clinically appropriate.🔬Liver Impairment
If starting in moderate hepatic impairment, reduce initial and titration doses by about 25%. In severe hepatic impairment, reduction may be about 50%, especially if ascites is present. Immediate-release formulations can be easier to fine-tune when dose adjustments are needed.💧Kidney Impairment
No routine dose adjustment is typically required in mild to severe kidney impairment, but half-life can vary widely in significant renal disease. Titrate cautiously and monitor tolerability.
