Infrared Light Therapy

Reviewed by the HeyPsych Medical Review Board

Board-certified psychiatrists and mental health professionals

Indications

Primary Indications

Chronic musculoskeletal painJoint stiffnessSoft tissue injury recoveryInflammation modulationAdjunctive post-stroke rehabilitation

Mechanism

Near-infrared light penetrates tissues and is absorbed by cytochrome c oxidase in mitochondria, enhancing ATP production, modulating reactive oxygen species, and promoting anti-inflammatory signaling. Far-infrared primarily induces vasodilation and increased blood flow through gentle heating.

Protocol

Preparation

Identify treatment area; ensure clean, dry skin; provide eye protection for near-infrared laser/LED.

Procedure

  1. Position device at manufacturer-specified distance.
  2. Apply near-infrared light for 10–20 minutes over target area (e.g., joint, muscle, scalp for neuro applications).
  3. For far-infrared, allow gradual warming over 15–30 minutes.
  4. Avoid continuous exposure to prevent overheating.

Frequency: 2–4 sessions per week initially; adjust based on response.

Duration: 4–8 weeks for initial course; maintenance as needed.

Total Treatment Time: Variable, often 8–20 sessions for musculoskeletal issues.

Equipment

  • Near-infrared LED or laser device (700–1,000+ nm)
  • Far-infrared heat panel or sauna
  • Eye protection for near-infrared

Expected Outcomes

Immediate

  • Mild warmth
  • Temporary pain relief

Short Term

  • Improved joint mobility
  • Reduced pain and stiffness

Long Term

  • Enhanced tissue healing
  • Sustained pain reduction
  • Improved circulation

Side Effects

common

  • Warmth
  • Mild redness

uncommon

  • Skin irritation

rare

  • Burn if used improperly

Contraindications

absolute

  • Active skin cancer on treatment site
  • Direct retinal exposure

relative

  • Pregnancy (avoid abdominal use)
  • Photosensitive conditions

special considerations

  • Monitor temperature in patients with impaired sensation.

Patient Selection

ideal candidates

  • Chronic pain sufferers
  • Post-injury recovery patients
  • Those seeking non-drug pain relief

screening required

  • Medical history review
  • Check for photosensitivity or cancer history at site

Research Evidence

Key Studies

  • Chow RT et al. Efficacy of low-level laser therapy in the management of neck pain: a systematic review and meta-analysis. Lancet. 2009.
  • Hamblin MR. Mechanisms and applications of the anti-inflammatory effects of photobiomodulation. AIMS Biophys. 2017.

Limitations

Variability in devices, dosing, and wavelength protocols; limited high-quality neurocognitive studies.

Cost Considerations

typical session cost: $30–$100 per session (clinic)

total treatment cost: $240–$800 for a typical course; $200–$1,000 for home units

insurance coverage: Often not covered for general wellness; sometimes covered for specific pain diagnoses.

cost effectiveness: Moderate to high when reducing medication use or improving function.
