Coherence Training

Reviewed by the HeyPsych Medical Review Board

Board-certified psychiatrists and mental health professionals

Indications

Primary Indications

Generalized anxiety disorderPost-traumatic stress disorderPerformance anxietyChronic stressDepression (adjunctive)

Mechanism

By providing real-time feedback on heart rate variability, coherence training helps individuals synchronize breathing and heart rhythms, shifting autonomic balance toward parasympathetic dominance and improving emotional regulation.

Protocol

Preparation

Baseline HRV assessment; instruction on diaphragmatic breathing.

Procedure

  1. Attach HRV sensor to ear clip, chest strap, or wrist device.
  2. Follow paced breathing cues (typically 5–6 breaths per minute).
  3. Use visual or auditory feedback to adjust breathing and emotional focus.

Frequency: 2–3 sessions per week initially, then maintenance as needed.

Duration: 4–8 weeks for core training; ongoing practice for lasting benefits.

Total Treatment Time: Variable; sessions last 20–30 minutes.

Equipment

  • HRV biofeedback device
  • Computer or mobile app
  • Breathing pacer (visual or auditory)

Session Structure

Pre-Session

Brief relaxation and device calibration.

Treatment Phase

Guided breathing and feedback exercises.

Post-Session

Discussion of progress; strategies for daily application.

Expected Outcomes

Immediate

  • Increased calmness
  • Improved breathing control

Short Term

  • Reduced anxiety
  • Lower stress reactivity

Long Term

  • Improved emotional regulation
  • Enhanced resilience

Side Effects

common

  • Mild dizziness
  • Sleepiness post-session

uncommon

  • Headache

rare

  • Hyperventilation if breathing too rapidly

Contraindications

absolute

  • Unstable cardiac conditions

relative

  • Severe panic disorder without concurrent therapy

special considerations

  • Modify breathing rate for respiratory conditions

Patient Selection

ideal candidates

  • Individuals with anxiety or chronic stress
  • Performance-focused clients
  • Trauma survivors (with support)

screening required

  • Cardiac safety screening

Training Requirements

practitioner

  • Training in HRV biofeedback protocols

facility

  • Access to HRV monitoring equipment and software

  • Tan G, et al. Efficacy of HRV biofeedback for PTSD symptoms: A pilot study. Appl Psychophysiol Biofeedback. 2011;36(1):27–35.

Limitations

Requires consistent practice; effects may diminish without maintenance.

Cost Considerations

typical session cost: $50–$150

total treatment cost: $300–$1,200 for multi-week program

insurance coverage: May be covered when billed as biofeedback for certain conditions

cost effectiveness: Moderate to high, especially if patient continues self-practice
