Carnitine (L-Carnitine, Acetyl-L-Carnitine)

Indications

Primary Indications

Mechanism

Carnitine transports long-chain fatty acids into mitochondria for β-oxidation, supporting ATP production. Acetyl-L-carnitine additionally donates acetyl groups for acetylcholine synthesis and crosses the blood-brain barrier, providing neuroprotective and neurotransmitter-modulating effects. These mechanisms may improve neuronal energy metabolism, enhance cognitive function, and reduce depressive symptoms.

Dosing

Adult Dosing

note: Often divided into 2–3 doses. Duration of 8–12 weeks studied in clinical trials.

typical: 500–2000 mg daily (L-carnitine); 500–1500 mg daily (Acetyl-L-carnitine)

Onset: 2–4 weeks. Duration: sustained with continued supplementation.

Adverse Effects

• Nausea
• Diarrhea
• Fishy body odor

⚠️ Serious Side Effects

  • Possible increased seizure risk in predisposed individuals

Warnings

Critical Safety Information

  • →May exacerbate fishy odor in patients with trimethylaminuria.
  • →Caution in seizure disorders.

Interactions

With: Valproic acid

Risk: May enhance carnitine depletion

Action: Supplementation sometimes recommended

With: Thyroid hormone

Risk: Carnitine may reduce thyroid hormone uptake

Action: Monitor thyroid function

Clinical Notes

  • Acetyl-L-carnitine has greater evidence for psychiatric and cognitive applications than plain L-carnitine.
  • May improve mitochondrial efficiency and reduce oxidative stress.
  • Meta-analyses suggest benefit for geriatric depression with fewer side effects than SSRIs.
