American Ginseng (Panax quinquefolius)

Indications

Primary Indications

Fatigue (general and cancer-related)Cognitive support (memory, attention)Adjunct for glycemic control in type 2 diabetesImmune support (traditional use)

Mechanism

Contains ginsenosides, which modulate the hypothalamic–pituitary–adrenal (HPA) axis, neurotransmitter systems, and glucose metabolism. Acts as an adaptogen, enhancing resilience to physical and mental stressors.

Dosing

Adult Dosing

fatigue: 200–400 mg/day divided

usual range: 200–1,000 mg daily of standardized extract (4–7% ginsenosides)

diabetes support: 200–3,000 mg/day (studied in divided doses)

cognitive support: 200–400 mg single dose or divided daily

Onset Duration

Acute cognitive effects reported within hours; fatigue and glucose control benefits may require weeks of use.

Adverse Effects

Common Side Effects

• headache
• insomnia
• GI upset
• nervousness

⚠️ Serious Side Effects

  • hypertension (rare)
  • hypoglycemia (in diabetic patients on medication)

Warnings

Critical Safety Information

  • →Use with caution in patients with diabetes; risk of additive hypoglycemia with medications.
  • →Can cause insomnia if taken late in the day.
  • →Avoid in uncontrolled hypertension.

Interactions

With: Antidiabetic medications (insulin, metformin, sulfonylureas)

Risk: Additive hypoglycemia

Action: Monitor glucose closely

With: Antihypertensives

Risk: May reduce efficacy of therapy

Action: Monitor BP

With: Stimulants (caffeine, ADHD meds)

Risk: Additive insomnia or nervousness

Action: Monitor

Monitoring

  • Blood pressure
  • Blood glucose (especially in diabetics)
  • Sleep patterns

Special Populations

👶Pregnancy

Not enough safety data; avoid use

🤱Breastfeeding

Avoid due to insufficient data

👧Children & Adolescents (Under 18)

Not recommended due to lack of safety data

👴Older Adults (65+)

May be beneficial in fatigue or cognitive decline; monitor for BP/glucose changes

Tapering

No formal tapering required; some traditional practices recommend cycling use.

Clinical Notes

  • American ginseng is chemically distinct from Asian ginseng, with different ginsenoside profiles and effects.
  • Some evidence supports use in cancer-related fatigue, especially in patients undergoing chemotherapy.
  • Often included in adaptogenic blends with rhodiola, ashwagandha, or schisandra.
