Published August 17, 2025•Updated February 20, 2026•Reviewed February 20, 2026

Indications

Primary Indications

Adjunct in major depressive disorder (particularly EPA-containing formulations)Prevention of perinatal depression (maternal supplementation)Cognitive health in aging and dementia preventionGeneral cardiovascular support

Mechanism

Algal omega-3 fatty acids (DHA and EPA) integrate into neuronal membranes, improving fluidity and signal transduction. EPA may exert antidepressant effects via anti-inflammatory pathways and modulation of cytokines. DHA supports neuronal development, synaptic plasticity, and neuroprotection.

Dosing

Adult Dosing

pregnancy: 200–300 mg/day DHA recommended

usual range: 250–1,000 mg/day combined DHA+EPA

depression studies: EPA ≥ 1,000 mg/day (though less common in algal products; often combined with DHA)

Dosage Forms

  • Softgels: 200–600 mg DHA (± EPA)
  • Liquid oils: variable concentrations

Onset Duration

Neuropsychiatric benefits may require 6–12 weeks of consistent use; cardiovascular benefits accrue over months to years.

Adverse Effects

Common Side Effects

• mild GI upset
• fishy or algae aftertaste
• burping

⚠️ Serious Side Effects

  • bleeding risk in combination with anticoagulants (rare)

Warnings

Critical Safety Information

  • →Caution with anticoagulant or antiplatelet therapy (warfarin, aspirin, clopidogrel).
  • →Algal sources are generally safer than fish oil for heavy metals, but quality varies.

Interactions

With: Warfarin or anticoagulants

Risk: Increased bleeding tendency

Action: Monitor INR or bleeding signs

With: SSRIs or other serotonergic drugs

Risk: Additive bleeding risk

Action: Monitor clinically

With: High-dose omega-3 (fish oil or other)

Risk: Additive GI side effects or bleeding risk

Action: Monitor dose and tolerance

Monitoring

  • Mood and cognitive changes
  • Signs of bleeding (easy bruising, nosebleeds)
  • GI tolerance

Special Populations

👶Pregnancy

Recommended source of DHA (200–300 mg/day); safe and commonly used.

🤱Breastfeeding

Safe; supports infant brain development via breast milk.

👧Children & Adolescents (Under 18)

Safe in age-appropriate doses; studied in ADHD and developmental support.

👴Older Adults (65+)

Safe and beneficial for cardiovascular and cognitive health.

Tapering

No taper required; can discontinue abruptly, though long-term use recommended for sustained benefit.

Clinical Notes

  • Algal oil is a vegan and sustainable alternative to fish oil.
  • DHA-dominant formulas are most common; EPA-dominant formulas are rare but may be more relevant for depression.
  • May be especially useful in perinatal populations where fish oil is less tolerated.
