Gamma Neurofeedback

Reviewed by the HeyPsych Medical Review Board

Board-certified psychiatrists and mental health professionals

Indications

Primary Indications

ADHD with attention and executive function deficitsMild cognitive impairmentPerformance enhancement in high-demand occupations

Mechanism

Trains voluntary modulation of gamma brainwave activity through operant conditioning with EEG-based real-time feedback, potentially enhancing neuronal synchrony and cognitive integration.

Protocol

Preparation

EEG baseline recording to determine individual gamma frequency targets.

Procedure

  1. Placement of EEG electrodes at target scalp sites
  2. Real-time feedback via visual or auditory cues
  3. Reinforcement when gamma activity meets target thresholds

Frequency: 1–3 sessions per week

Duration: 30–60 minutes per session

Total Treatment Time: 8–40 sessions depending on goals and progress

Equipment

  • EEG amplifier and electrodes
  • Computer with neurofeedback software
  • Visual and/or auditory feedback interface

Session Structure

Pre-Session

Setup of EEG equipment and baseline check.

Treatment Phase

Active neurofeedback training with reward-based cues.

Post-Session

Review of performance metrics and progress.

Expected Outcomes

Immediate

  • Increased awareness of brain activity patterns

Short Term

  • Improved focus and mental clarity

Long Term

  • Sustained attention improvements
  • Enhanced working memory

Side Effects

common

  • Mild fatigue
  • Transient headache

uncommon

  • Irritability
  • Sleep pattern changes

rare

  • Provocation of seizure in susceptible individuals without clearance

Contraindications

absolute

  • Uncontrolled epilepsy

relative

  • Severe anxiety or mania that could be worsened by stimulation

Patient Selection

ideal candidates

  • Individuals with attention deficits or seeking cognitive enhancement

screening required

  • EEG assessment
  • Clinical evaluation

Training Requirements

practitioner

  • Licensed neurofeedback clinician
  • Neuropsychologist

facility

  • Neurotherapy clinic
  • Research lab with EEG capability

  • Escolano C, et al. Gamma neurofeedback training in healthy participants: effects on cognitive performance. Front Hum Neurosci. 2014;8:436.

Limitations

Small sample sizes; heterogeneity in protocols.

Cost Considerations

typical session cost: $75–$150

total treatment cost: $1,200–$4,500 depending on sessions

insurance coverage: Generally not covered for cognitive enhancement; possible coverage for ADHD under neurofeedback.

cost effectiveness: Potentially cost-effective for motivated individuals seeking long-term improvement.

Clinical Notes

  • Baseline EEG mapping is recommended before starting.
  • Protocols may target specific brain regions depending on goals.
